Your personal and medical information will be closely guarded and anything you disclose, either written or verbal, will be between you and the licensed massage therapist ONLY.
Draping is a state law in Tennessee, and you agree to be completely draped for the duration of the massage. Understanding that if you are uncomfortable temperature wise, you should inform the therapist and they will make the appropriate changes to the draping to ensure your comfort.
Massage should not be used as a substitute for medical treatment, and you understand that your massage therapist is not qualified to diagnose medical or mental ailments, and nothing they say should be construed as such.
The massage therapist is not trained or licensed to make spinal adjustments.
Massage should not be performed under certain medical conditions and you should truthfully complete the intake form provided by the therapist. And it is your responsibility to keep the therapist updated on any changes in your medical history or prescriptions. You understand that there is no liability on the therapist’s part if you should fail to do so.
It is your responsibility to inform the therapist of any needed adjustment in pressure during the massage.
Cancellation Policy:
I ask that you provide 24-hour notice of any schedule change or cancellation requests. Please understand that when you cancel or miss your appointment without providing a 24-hour notice, I am often unable to fill that appointment time, this is an inconvenience as I have reserved that time specifically for you. For this reason, you will be sent an invoice for the session fee to be paid before scheduling your next appointment and I reserve the right to require a credit card be put on file to book future sessions.
I understand that emergencies can arise, and illnesses do occur at inopportune times. If you have a fever, known infection, or experienced vomiting or diarrhea within 24-hours prior to your appointment time I request that you cancel your session immediately. Late cancellations due to emergency or illness will generally not result in any charges, provided you notify me as soon as possible - but this is determined on a case-by-case basis.
Late Arrival Policy:
You understand that issues can arise that may cause you to be late for your appointment. However, I ask that you call to inform me as soon as possible if this ever occurs so I can do my best to accommodate you. Appointment times are reserved for each client and arriving after your appointment time may result in a shortened session so that your massage can end at the scheduled time and you will be responsible for the full fee of your scheduled session.
In return, I will do my best to be on time and if I’m unable to do so, I will add time to your session or adjust the service charge accordingly.
Inappropriate Behavior Policy:
Understand that your massage therapy session is for therapeutic and relaxation purposes ONLY. There is absolutely NO sexual component to this massage session whatsoever. Any insinuation, joke, gesture, or request otherwise will result in IMMEDIATE TERMINATION of your session and a refusal of any and all services in the future. You will be charged the full session fee regardless of the length of your session. And depending on the behavior exhibited, I may also file a report with the local authorities if necessary.
